About UIF Bank Form
If you lose your work, your first point of action should be to file a claim with the Unemployment Insurance Fund. The Department of Labour’s uFiling portal, which allows “workers [to] use the system to apply for benefits such as unemployment, death, and illness,” is the quickest and most simplest way to do this.

Please keep in mind that this is also the procedure to take if you are facing decreased work hours or a temporary layoff as a result of the COVID-19 outbreak. In such cases, your employer can also assist you with the application. If you do not have internet access, you can submit your claim(s) by e-mail or fax.
The applicable UIF forms are:
- UI-19 and UI-2.7 (completed by Employer)
- UI-2.1 (application form)
- UI-2.8 (bank form completed by the bank)
- A letter from your Employer confirming Reduced Work Time or Temporary Lay-off due to the Coronavirus
- Copy of your ID document
What about claiming an illness benefit?
If you are sent home to self-quarantine for 14 days due to a positive COVID-19 diagnosis and are unable to work from home, you have the right to claim for an illness benefit for that time period.
These are the forms and documents you need to complete and submit:
- UI-19 and UI-2.7 (completed your employer)
- UI-2.2 (a portion of which needs to be completed by a medical doctor)
- UI-2.8 (completed by your bank)
- Copy of ID document
- Employer Declaration (Covid19)
- Employee Declaration (Covid19)
What do I need to claim for a death benefit?
If tragedy happens and a breadwinner is gone, the families of deceased employees are also eligible to request for financial assistance.
The required documents are:
- UI-19 and UI-53 (completed by the deceased’s employer)
- UI-2.5 or UI-2.6
- Death Certificate
- ID of deceased and applicant
- UI-2.8 (bank form completed by the bank)
- Copy of ID document

Pay Your Unemployment Insurance Fund Account
Add your bank account
- Log into uFiling Website.
- Select “Employer Information” and the “Banking Information”.
- Complete the required banking Information.
Pay your account
- Log into uFiling Website.
- Select “Statement of Account” from your home page.
- Select the declaration or declarations you wish to pay.
- Click “Pay Accounts”.
- Choose the FNB bank account type you wish to pay from: FNB-CAMS, FNB Online Banking and FNB Internet Banking.
FNB Online Banking Enterprise
- Log into Online Banking Enterprise.
- Navigate to the “Payments” tab.
- Click on “eFiling” sub tab.
- Click on “Submit eFiling” in the menu to submit a uFiling payment authorisation.
- Select the eFiling payment which requires authorisation and click on “Continue”.
- The landing page will no longer reflect the eFiling payment as it has been forwarded for authorisation.
- Click on the “Payments” sub tab.
- Click on “Authorisation Needed” and select the uFiling payment to authorise. Thereafter click on “Authorise”.
- Enter your Digital Certificate and password and click on “Accept”.
